Key Job Duties And Responsibilities
The Licensed Practical Nurse is responsible for providing direct nursing care to residents in accordance with federal, state, and local standards and regulations, as required by the Director of Nursing Services or Nurse Supervisor to ensure the highest degree of quality care is maintained at all times.
The following is a general list of key duties and responsibilities in your position. This list is not exhaustive, and you will be required to perform other duties as assigned by an authorized supervisor.
- Comply with the Facility’s personnel, safety, and corporate policies and procedures.
- Comply with the nursing department’s policies and procedures.
- Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing, or Unit Manager.
- Treat residents, family members, visitors, and team members with dignity and respect.
- Maintain the confidentiality of all resident and family information.
- Report to work on time and as scheduled.
- Assist in the admission and discharge of residents.
- Assist residents in daily hygiene care including bathing, oral, ear, eye, facial, hair, and nail care.
- Keep resident’s clothing and linen dry and replace when soiled or wet.
- Assist residents with bowel and bladder functions; collect specimens when required.
- Assist in transporting residents to and from appointments, activities, and social programs.
- Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
- Report and document any changes observed in a resident’s condition or behavior to the Charge Nurse; assist in the care of dying residents and provide post‑mortem care.
- Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, colostomy and drainage bags, and taking blood as required.
- Perform lab tests, blood pressures, and other routine monitoring as required by the resident care plan; admit, transfer, and discharge residents as needed.
- Complete accident/incident reports and submit them to the Director as required.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting as necessary.
- Fill out and file required record‑keeping forms and charts upon the resident’s admission, transfer, and/or discharge.
- Chart nurses’ notes in an informative and descriptive manner that reflects the care provided and the resident’s response.
- Complete required documentation of care and services as they are provided.
- Review complaints and grievances filed by residents and make appropriate reports to the Nurse Supervisor.
- Receive and give the nursing report upon reporting in and at the end of shift duty hours.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals according to facility policies.
- Monitor assigned personnel to ensure adherence to safety regulations in the use of equipment and supplies.
- Recommend to the Nurse Supervisor the equipment and supply needs of your unit/shift.
- Review care plans daily to ensure appropriate care is being rendered.
- Maintain confidentiality of all resident care information.
- Oversee resident needs, staff, and operations as needed.

- Valid professional license (LPN) in good standing with the State.
- Valid CPR certification.
- Basic computer skills; able to read, write, speak, and understand English.
- Ability to work beyond normal hours, including weekends and holidays when necessary.
- Ability to assist in the evacuation of residents during emergency situations.
- Ability to perform essential position functions with or without reasonable accommodation.
- Use of a transfer/gait belt or obtain assistance when lifting or carrying objects in excess of 25 pounds.
