Position Summary

Assists nursing personnel in providing non-professional nursing care and simple technical nursing services under the direction and supervision of an RN, LPN or CMA.

Performance Standards
  • 1. Follows established performance standards and implements facility P & P
  • 2. Implements the resident Bill of Rights
  • 3. Attends facility education in services / staff meetings
  • 4. Assumes responsibility for compliance with federal, state and facility regulations
  • 5. Documents I & O, Meal intakes, BM’s, behaviors, ADL’s, position changes, weights and vital signs accurately and timely.
  • 6. Reports changes in resident condition to charge nurse immediately
Functions
  • 1. See CNA Job Procedures for all shifts
  • 2. Attends report daily.
  • 3. Conducts rounds on residents every 2 hours and notes any changes to charge nurse.
  • 4. Answers call lights of residents within 5 minutes, answering emergency call lights first.
  • 5. Performs care according to CNA Job Procedures
  • 6. Offers fluids throughout day to residents
  • 7. Responsible to see all residents get to meals on time.
  • 8. Promotes good body alignment for the resident
  • 9. Monitors environment for possible safety hazards
  • 10. Transfers residents to and from activities, meals, beauty shop, etc.
  • 11. Assists residents with toileting as per care plan direction
  • 12. Follows resident care plan for all aspects of care
  • 13. Gives recommendations to optimize resident safety
  • 14. Gives recommendations to improve resident quality of life
  • 15. Prevents, protects and reports abuse, neglect and /or exploitation
  • 16. Assists with restorative and rehabilitative care
  • 17. Promotes and respects resident rights
  • 18. Answers telephone appropriately
  • 19. Runs errands as directed by the charge nurse, DON, ADON or Care Coordinator
  • 20. Charts required information as indicated under Performance Standards
  • 21. Checks restraints according to policy
  • 22. Measures, records and reports to charge nurse the vital signs
  • 23. Collect specimens such as UA’s, BM’s
  • 24. Empty’s catheters and measures
  • 25. Empty, rinse and make sure commodes are kept clean
  • 26. Practices correct infection control measures and isolation techniques
  • 27. Keeps temperature of resident room between 68 - 72

    F.
  • 28. Provides appropriate music / activities to residents
  • 29. Notifies maintenance of any equipment repairs, lights out, safety hazards requests of residents
  • 30. Notifies Social Services of any resident needs, requests
  • 31. Notifies Care Coordinator of any care concerns / issues
  • 32. Notifies Corporate Compliance Officer of any concerns.
  • 33. Controls odors by taking out trash immediately, toileting / changing soiled residents immediately, flushing toilets, washing out commodes, good hygiene for self and resident, airing rooms out appropriately, spray disinfectants appropriately
  • 34. Follows Employee handbook
  • 35. Performs other duties as directed by supervisors
Working Conditions
  • Must be able to lift 50+ pounds
  • Breaks: one 15 minute break and one 30 minute lunch break during the 8 hour shift
  • Well lighted, well-ventilated halls and rooms
  • Stands, sits, walks throughout the workday.
  • Subject to frequent interruptions
  • Works up to 8 hours a day under hourly wage and is expected to work beyond normal working hours when necessary.

  • Employee will access, use, and disclose protected electronic and other resident health information
    * only as authorized in this job description or as authorized by your supervisor.
  • Employee will conduct any oral discussion of resident protected health information
    * with other staff or with residents and family members in a confidential manner and/or setting.
  • Employee will report suspected violations by any person obligated to safeguard resident protected electronic or other health information to your supervisor or security officer immediately.
