The Role You’ll Play as a Crisis Case Manager

As a Crisis Case Manager, you’ll be a vital part of our community‑based crisis response team, working alongside a clinician or peer support specialist to provide timely, face‑to‑face support for individuals experiencing a mental health crisis. You’ll meet people where they are—whether that’s at home, work, or in a public space—to de‑escalate, stabilize, and connect them with services that can prevent hospitalization, arrest, or emergency room visits.

You’ll Do
  • Provide immediate, in‑person crisis intervention and de‑escalation in community settings
  • Conduct triage and coordinate with clinicians, first responders, and community partners
  • Assist clients in safety planning and connect them to appropriate care and recovery resources
  • Follow up with clients to ensure continuity of care and support
  • Document services accurately and on time in the electronic health record
  • Transport clients and travel to service locations using company vehicles
  • Engage in supervision, training, and collaborative team efforts

Requirements
  • Bachelor's degree from an accredited college or university with a major in gerontology, nursing, health, social work, counseling, human development, or family studies, or related area approved by KDADS SCA/OAA Program Manager, and that individual has at least one (1) year experience in the aging and/or disability field; or
  • A Registered Professional Nurse licensed to practice in the State of Kansas with at least one (1) year experience in the aging and/or disability field
  • Strong oral and written communication skills using the English language.
  • Basic personal computer skills which enable the entry, retrieval and use of electronic data.
  • A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.
  • Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services.
Preferred
  • Knowledge of the needs and challenges of adults and children who experience Severe and Persistent Mental Illness and Severe Emotional Disturbance.
  • Experience with Microsoft Office software.
