Job Summary
The Case Management Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
Major Duties- The Social Work and Nursing Case Management Service Line is a multidisciplinary team that addresses the healthcare needs of veterans receiving Inpatient Care, Outpatient Primary and Specialty Care, as well as Extended Care Services. As a member of the interdisciplinary team, you will participate in the development and application of directed clinical treatment interventions. Functions of this position include, but not limited to:
Evaluates care and outcomes to ensure timely and appropriate provision of services. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care, patient and family education, patient self-management, and supporting factors that impact customer satisfaction. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.

Qualifications & RequirementsEducation & Requirements: U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
