Overview
The Case Management Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
Responsibilities- The Social Work and Nursing Case Management Service Line is a multidisciplinary team that addresses the healthcare needs of veterans receiving Inpatient Care, Outpatient Primary and Specialty Care, as well as Extended Care Services.
- As a member of the interdisciplinary team, you will participate in the development and application of directed clinical treatment interventions.
