Complete assessment and nursing care in accordance with quality standards. Provide initial and ongoing nursing assessment and appropriate care based on clinical and professional judgment.

ESSENTIAL JOB FUNCTIONS
  • Complete an initial assessment of the participant and family to determine home care needs, including a complete physical assessment and history of health issues; determine nursing diagnosis.
  • Reassess participant health and nursing needs as required and provide health care instructions.
  • Develop a nursing care plan that identifies goals and establishes therapeutic, preventive, and rehabilitative nursing actions; ensure participant and family involvement in the planning process.
  • Implement appropriate therapeutic, preventive, and rehabilitative nursing procedures as outlined in the plan of care; make necessary revisions as participant status and needs change.
  • Provide counsel to the participants in meeting health needs.
  • Develop and implement discharge planning as part of the plan of care.
  • Ensure that all services are delivered consistently with the standard practice of professional nursing.
  • Document contacts and needed follow-up information accurately, thoroughly, and within agency, funding source, and regulatory authority timelines.
  • Participate in certification and regulatory authority surveys as assigned.
  • Complete special projects assigned by the supervisor.
  • Provide nursing services to participants consistent with established professional nursing practices, state and federal licensing regulations.
  • Exercise professional judgment to assess participant needs, evaluate and plan for meeting care needs, and implement the authorized Plan of Care.
  • Make home visits to participants to provide nursing services.
  • Prepare clinical notes and update the primary physician as necessary and at least every 60 days.
  • Effectively communicate with the physician regarding the participant's needs and report changes in the participant’s condition; obtain/receive physician orders as required.
  • Ensure that equipment and other necessary items/services are available in a timely manner.
  • Instruct, supervise, and evaluate home health aide care every 14 days.
  • Participate in On-Call rotation as assigned.
  • Conduct marketing activities under the direction of the supervisor.
  • Conduct quality control of Oasis entries as directed by the supervisor.
  • Regular and predictable attendance.
REQUIRED SKILLS
  • Bachelor’s degree with one year of experience in home health preferred.
  • Satisfactory background check.
  • Licensed in the State of Missouri as a Registered Nurse.
  • Must maintain current CPR certification.
  • Ability to travel.

WORK ENVIRONMENT & PHYSICAL DEMANDS
  • May be exposed to extremes of heat and cold weather conditions. Must be able to travel during various inclement weather conditions.
  • May be exposed to infections and contagious diseases. Contact with participants under a wide variety of circumstances. May be exposed to participant elements. Subject to varying and unpredictable situations.
  • OSHA exposure Category I - includes tasks that involve exposure to blood, body fluids, and tissue conditions.
