• Provide program orientation to patients, families, and caregivers, covering the case manager’s role, rehab philosophy, stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and patient rights and responsibilities.
• Act as coordinator of patient/family/caregiver education; monitor documentation of education by team members and promote participation of the patient/family/caregiver in team discussions related to goals and status through family conferences and other interactions.

• (RN) Registered Nurse
• Associate Degree, or Registered Nurse Diploma
