The Role

The PRN Registered Nurse Case Manager plans, organizes, and directs hospice care with an emphasis on community health education/experience. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.

Job Responsibilities
  • Patient Care
  • Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs, including a complete physical assessment and history of current and prior illness.
  • Provides professional nursing care using all elements of the nursing process.
  • Assesses and evaluates patient status, writes and initiates a plan of care, regularly re-evaluates needs, and revises the plan as necessary.
  • Initiates and adjusts the plan of care as patient status and needs change.
  • Uses health assessment data to determine nursing diagnoses and develops goals in a care plan that includes the patient and family in the planning process; administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate.
  • Assists the patient with activities of daily living and supports self-sufficiency and comfort care.
  • Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care for an assigned caseload.
  • Participates in on-call duties as defined by policy.
  • Communication
  • Completes and submits accurate clinical notes; records pain and symptom management changes/outcomes.
  • Communicates with the physician regarding needs and reports changes in condition; coordinates orders as required.
  • Coordinates with community health-related persons to implement the care plan; teaches self-care techniques to patient and family; collaborates with the interdisciplinary group; maintains a safe environment for the patient.
  • Attends interdisciplinary group meetings.
  • Additional Duties
  • Participates in on-call duties; ensures equipment and services are available; supervises ancillary personnel as required.
  • Engages in ongoing professional development and continuing education; fulfills case assignments; participates in quality assessment and performance improvement activities.
Job Qualifications
  • Graduate of an accredited school of nursing. One to two years of recent acute care experience and one to two years in home care preferred; hospice care experience preferred.
  • Current licensure in state and CPR certification.
  • Bachelor’s degree preferred, with one year of hospice care experience.
  • Management experience is not required; responsible for supervising hospice aides.
  • Excellent observation, communication, and problem-solving skills; nursing skills per competency checklist.
  • Physical ability to lift/move patients and equipment as required; able to walk/stand for extended periods.
  • Must be a licensed driver with a valid, insured vehicle in good working order.
