Overview
The Care Facilitation Coordinator provides administrative support and works closely with other areas of the Care Integration Department by entering authorizations for selected services, distributing faxes and tasks to appropriate team members, screening patients for care integration programs, and other supportive services to meet the needs of the population.

Responsibilities- Process inpatient and outpatient prior authorization requests.
- Performs core administrative functions and support to the care teams.
- Support functions of the care team required to manage the assigned population.
- HS diploma or equivalent and 3-5 years of experience in clinical settings such as hospital or physician's office, and/or experience in managed care or Medicaid health plan.

Starting PayThe pay range for this job begins at $21.70/hr; offers are determined based on education and experience.
