Overview
Provides Community Support services to youth diagnosed with serious and persistent mental health needs and significant impairments in functioning. Services are community-based and focused on helping youth and families achieve individualized life goals within their living environment. Hours are flexible and may include evenings to meet the needs of the youth and families Beacon Mental Health serves.
Essential Duties And Responsibilities- Provides community-based services to an assigned caseload, using independent judgment to determine appropriate treatment.
- Provides crisis intervention supports to consumers experiencing a psychiatric or substance-related crisis, including safety planning or referrals for hospitalization or additional mental health treatment as needed.
- Maintains ongoing contact with hospitalized clients and participates in staffing and discharge planning to support successful reintegration into the living environment.
- Completes documentation in an electronic health records system within three business days of services provided, clearly reflecting interventions and medical necessity.
- Coordinates care for consumers with treatment providers and other agencies.
- Develops person-centered treatment plans initially and annually that are strengths-based and reflect the consumers and family s life goals and choices according to DMH/agency timelines.
- Collaborates with the Health Care Home team to support clients overall physical health using wellness coaching techniques.
- Completes initial and annual Crisis and Wellness Plans, Client Rights and Responsibilities, and other required paperwork in a timely manner.
- Maintains knowledge of agency technology programs/systems needed to perform the role and uses them effectively.
- Maintains required training and certifications and adheres to agency productivity standards.
- Performs other duties and responsibilities as assigned by the supervisor.
- Master’s degree preferred;
Bachelor’s degree considered with relevant experience. - Active driver’s license and minimum state-required auto-insurance.
- Reliable transportation required.
- CPR/First Aid Certification required.
- 1 to 3 years of prior work-related experience preferred.
- Knowledge of behavioral health diagnoses and symptoms.
- General knowledge of community resources and willingness to research as needed.
- Strong crisis management skills
- Excellent written and verbal communication
- Ability to use and troubleshoot basic business equipment
- Ability to interact with a wide range of individuals
- Strong multitasking ability with high accuracy and timeliness
- Ability to maintain client confidentiality
- Problem solving and task prioritization for efficiency
