The Healthcare Home program integrates care for chronic health conditions within a Community Mental Health Center setting. The program supports individuals in accessing needed health services, managing chronic conditions, and improving overall health through ongoing monitoring, early intervention, and care coordination. Healthcare Home promotes wellness, healthy lifestyles, and preventive care while using a population health approach to improve outcomes for the individuals served.
WhatYou’ll Do
- Manage a caseload of clients, providing primarily in-person support with occasional phone follow-ups.
- Coach individuals and groups on health and fitness goals through personalized guidance and motivation.
- Lead educational sessions on topics like nutrition, fitness, and tobacco cessation.
- Transport clients to off-site appointments and activities using your personal vehicle as needed.
- Oversee on-site exercise programs.
- Engage referred clients and encourage participation in the Health Club.
- Document client interactions in the electronic medical record system.
- Collaborate with internal teams and community partners to care and support client wellness.
- Build and maintain relationships with local gyms and wellness resources.
- Bachelor’s degree in a related field (e.g., social science, health and wellness) preferred.
- Certification as a personal trainer, nutritionist, or dietitian preferred.
- Valid Missouri driver’s license and proof of automobile insurance; must meet safe driving criteria.
- Knowledge of mental illness and substance use disorders.
- Strong customer service skills with a focus on recovery support.
- Excellent written, verbal, and interpersonal communication.
- Proficient with Microsoft Word, Excel, and email.
