The Refugee Services Case Manager provides comprehensive, strengths-based case management targeting refugee and immigrant populations. The Case Manager will focus on serving walk-in clients, coordinating health and mental health navigation services, employment, school navigation and housing support, and intensive services for vulnerable populations including seniors. The Case Manager will promote client self-sufficiency through direct service, advocacy, resource navigation, and referrals in alignment with Catholic Charities’ mission and the guidelines set by agency standards and any funder requirements.

Essential Functions
  • Provide trauma-informed and culturally competent case management to eligible individuals, including newly arrived refugees, asylees, humanitarian immigrants and other vulnerable population.
  • Assess client needs, develop Family Self-Sufficiency Plans (FSSPs) or Self-Sufficiency Plans (SSPs), and provide ongoing support to achieve individual goals in housing, employment, medical care, and social integration.
  • Ensure client understanding of rights, responsibilities, and available services within offered community supportive programming.
  • Provide timely, strengths-based response to walk-in clients, addressing urgent needs through assessment, referrals, and short-term follow-up.
  • Collaborate with internal teams and external partners to resolve client crises efficiently, while maintaining professional boundaries and ethical standards.
  • Offer navigation support for complex health systems, including assistance with health insurance enrollment, Medicaid, and public benefits.
  • Coordinate with medical providers and community health workers to support appointments, transportation, interpretation, and adherence to treatment.
  • Facilitate or support wellness groups and workshops on public safety, parenting, stress management, and other health-related topics.
  • Identify barriers to employment, assist clients in job readiness, and facilitate job placements.
  • Support clients in locating and maintaining stable housing, connecting them with available housing resources, rent assistance, and landlord advocacy when needed.
  • Provide intensive case management to especially vulnerable populations including survivors of trauma, individuals with mental health conditions, seniors, and those at risk of homelessness.
  • Advocate for clients with complex needs, ensuring accessibility through language services, transportation support, and coordination with community partners.
  • Maintain accurate and timely case notes, service records, assessments, and referrals in the designated electronic case management system.
  • Follow all grant and agency guidelines for program implementation.
  • Participate in ongoing training, including VIRTUS, trauma-informed care, and mandated reporting procedures. Follow all agency protocols and NASW Code of Ethics.
Additional Responsibilities
  • Conduct home and field visits in accordance with health and safety protocols.
  • Contribute to program and grant reporting as needed.
  • Assist with outreach, community engagement events, and client workshops.
  • Perform other duties as assigned by supervisor or agency leadership.
Requirements

MINIMUM POSITION REQUIREMENTS:

  • Bachelor’s Degree in social work or human service field or comparable experience is preferred
  • Must have competency in oral, written, and interpersonal communication skills in English; fluency in a second language (Burmese, Karen, Dari, Swahili, etc.) preferred
  • Ability to work effectively in a multi-cultural and multi-lingual environment
  • Strong typing and internet skills as well as the ability to learn Microsoft Outlook, Excel and various database programs
  • Must be able to work collaboratively with outside agencies and advocate on behalf of the clients
  • Must be reliable, punctual, professional, and respond appropriately to people in crisis
  • Must have a valid driver’s license, car insurance, a verified safe driving record and reliable means of transportation for local travel; must provide a copy of license and insurance
