Overview
Ambulatory Clinic Nurse Care Coordinator (RN) – Urology, Days, Full Time. The Ambulatory Clinic RN Care Coordinator is a licensed registered nurse who is a clinical expert delivering ethical, evidence-based, safe and compassionate nursing care. The RN Care Coordinator focuses on patient safety and quality of nursing care by identifying and clarifying patient needs, performing clinic-related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes.
They provide support to assigned physicians by communicating with the patient/family/caregiver and relaying information to the physician. They support nursing care services during the physician's clinic sessions and ensure the patient/family/caregiver understands the plan of care. They verify that orders placed during the clinic session have been completed and that the physician is notified of the results.
- Responsible for management of patient flow, triage, and direct patient care.
- Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager.
- Manage the care of the patient population by developing expected patient outcomes, defining appropriate resource utilization, and working with the provider to set patient goals.
- Provide nursing support/coordination to the practice to optimize patient outcomes and improve the patient’s quality of life through clinical and educational programs and/or support services.
- Support patient self-management of disease.
- Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
- Manage high-risk patient care, including patients with multiple co-morbidities or high risk for readmission, including a registry.
- Develop relationships with patients and competently use assessment skills to identify physical, psychosocial, financial, and environmental health care needs.
- Educate the patient and appropriate ancillary personnel about health care needs, safety issues and benefits including patient education.
- Utilize available electronic tools, such as EMR, to document assessments, clinical findings, plans of treatment and progress toward goals.
- Observe, record and report patient condition and reaction to drugs and treatments to physicians. Educate patient/family about diagnostic procedures, medications, nutrition and maintenance of health and wellness.
- Maintain/review patient records, charts and other pertinent information. Record tests and examination results and follow up with patient. Post tests and examination results.
- Respond to/refert incoming patient treatment-related phone calls. Instruct patient and family regarding medications and treatment instructions.
- Maintain patient confidentiality.
- In providing patient care, document using EMR to maintain and/or develop patient records in compliance with practice standards.
- Must be able to perform the professional, clinical and/or technical competencies of the assigned unit or department.

Education and Experience
- High School Graduate
- Bachelor Degree in Nursing
- 2 or more years of RN experience.

- Previous medical office experience.
- Basic Life Support Heartsaver (BLS) - American Heart Association (AHA) or obtained within 14 days
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
- Must have current active multi-state Registered Nurse (RN) license in good standing in the state in which you live OR active single-state RN license in good standing in the state you will be working, with proof of application of a multi-state RN license in the state in which you live. Staff hired prior to 2/1/2024 are required to obtain a multi-state RN license and renew by 3/31/2026.
