Position Title:
Transplant Nurse Coordinator – Liver Bell Hospital
Position Summary:
The Organ Transplant Nurse Coordinator plans, directs, and provides specialized nursing care with the primary emphasis on the treatment of individuals who have advanced or end stage disease and are in need of a transplant. Provides continuity and coordination of patient care; conducts patient and professional education initiatives; collaborates with multidisciplinary teams, community resources and agencies.
- Demonstrate ability to provide comprehensive management of end-stage organ diseases and oversee the patient’s journey through the transplant process.
- Coordinate living donation and evaluate the living donor for assessment, provide education, and maintain documentation.
- Coordinate the care of transplant recipients in the outpatient setting, address physical, social, emotional rehabilitation, provide post‑operative teaching, collect data, communicate lab results, and arrange consultations.
- Monitor and coordinate care of candidates awaiting organ transplantation, maintain candidate listing status, and provide documentation.
- Maintain and update candidate listing status, identify re‑evaluation needs, and maintain health maintenance and malignancy screening.
- Continuously grow knowledge of transplantation, review current literature, attend conferences, participate in professional organizations, mentor new staff, and assume responsibility for high quality care.
- Facilitate evaluation of potential transplant candidates, identify requirements, and coordinate multidisciplinary evaluation.
- Manage post‑transplant medication, complications, interventions, discharge planning, collaborate with team, monitor progress, and communicate status.
- Educate candidate/family/legal guardian about living donation options, regulations, organ allocation, waiting phase, transplant process, risks/benefits, and post‑operative therapy.
- Coordinate transplant process when organ becomes available, adhere to regulations, communicate decision data, and maintain communication with donor and recipient teams.
- Identify psychosocial and economic needs of candidate/family/legal guardian, comply with policies, and educate at listing time.
- Interpret serological results of potential transplant candidate, evaluate contraindications, and educate about immunizations required prior to transplantation.
- Perform professional, clinical, and technical competencies of assigned unit or department.

Education and Experience
- Bachelor’s Degree in Nursing
- 2 or more years acute clinical experience
- Licensed Registered Nurse (LRN) – Multi‑State – State Board of Nursing in Kansas
- Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) or obtained within 14 days
- Active multistate RN license in good standing in state where you live OR active single state RN license in good standing in the state you will be working with proof of application of a multistate RN license in the state you live.
