Employment Type: Full Time | Monday - Friday, 8a-5p
Pay Rate: $30+ / hour (based on requirements and experience)
Location: Kansas City, KS
Sign on Bonus: $2,500
We are seeking a caring and dependable Community RN to support our participants both in the center and in their homes. The Community RN plays a key role in promoting the health and well-being of participants through hands‑on nursing care, ongoing assessments, and collaboration with the care team. This position is ideal for a nurse who enjoys building relationships, working in community‑based care, and supporting older adults in living safely at home.
Key Responsibilities- Support Case Managers in implementing care plans and monitoring participant health status
- Monitor for changes in condition and report promptly to the provider
- Assist in coordinating services, medical appointments, and follow‑up care
