Responsibilities

Essential Duties and Responsibilities (includes, but not limited to):

* Provide direct patient care, including hygiene support, mobility assistance, and comfort measures.

* Monitor vital signs, document observations, and report changes in patient condition.

* Administer medications as prescribed and observe for reactions or side effects.

* Perform wound care, dressing changes, and assist with procedures within state LPN scope.

* Collaborate with RNs, physicians, and the care team to support individualized care plans.

* Assist with discharge planning and provide patient/family education on treatments and recovery.

* Follow infection control practices and maintain accurate patient records.

Qualifications

Before we go any further, we do have some deal-breakers. You must have:

* Current and valid state license

* Current BLS certificate upon hire and maintain current

* IV certification upon hire (state specific)
