The MDS Coordinator is responsible for overseeing the completion and accuracy of all MDS assessments in accordance with state and federal regulations. This role ensures timely and compliant PPS and OBRA assessments, coordinates care plans with the interdisciplinary team, and supports accurate reimbursement under PDPM. The MDS Coordinator works closely with nursing leadership to maintain survey readiness, quality measures, and clinical documentation standards.
Qualifications- RN or LPN license (per state and facility requirements)
- MDS 3.0 and PDPM experience preferred
- Strong knowledge of Medicare regulations and reimbursement
- Skilled nursing facility experience required
