Job TitleHospice Chaplain (per diem) – Kansas City, KS market
Job SummaryThe Hospice Chaplain, under the supervision of the Clinical Director, works as an integral part of an interdisciplinary team and provides spiritual, emotional and bereavement care to support hospice patients, clients, and families. The Chaplain must possess knowledge of and commitment to hospice philosophy of care.
Responsibilities- Provide pastoral/spiritual care to patients and/or families.
- Visit and minister to patients; develop a spiritual care continuum of care with the Clinical Director and other team members.
- Conduct spiritual assessments and develop care plans addressing physical, emotional and spiritual needs and goals.
- Provide pastoral support at bedside, funerals, and memorials as requested.
- Document visits per company policy and compliance requirements.
- Function as a member of the interdisciplinary team.
- Organize company-sponsored memorial services as appropriate.
- Provide emotional/spiritual support to staff and participate in staff meetings and Interdisciplinary Team meetings.
- Initiate and maintain communication with area inter-faith clergy.
- Facilitate spiritual care, acting as a bridge between patients, families and clergy to provide pastoral care and end-of-life support as requested.
- Educate staff about religious needs and issues affecting care and treatment.
- Perform other duties as assigned; report to and be accountable to the Clinical Director.
- Collaborate with the interdisciplinary team to ensure holistic patient care and contribute to well-being of clients, patients, families and staff.
- Master’s of Divinity Degree or equivalent in theology or related field.
- At least four units of Clinical Pastoral Education (CPE);
Board Certified or eligible for Board Certification under ACPE or CPSP. - Excellent communication and pastoral skills; ability to support patients and families who experience pain, suffering, death and grief; strong organizational and decision-making skills.
- Computer literacy and timely charting in the company’s EMR system.
- Certification or eligibility for CPSP or ACPE certification.
- Minimum of two years of pastoral experience in a healthcare setting; hospice experience is desirable; ability to support patients/families from diverse faith traditions.
- Strong interpersonal skills; ability to work with diverse ages, cultures, backgrounds, beliefs and values.
- Professional knowledge to develop and maintain collaborative relationships with the healthcare team.
- Ability to work independently, take initiative and be accountable; good physical and emotional health.
- Endorsement or ordination in a religious setting is desirable.
