Associate Medical Director – Center Well Senior Primary Care – Kansas City
Base pay range: $246,100 – $344,200 per year. Salary is based on experience, skills and education. The role is eligible for a bonus incentive plan.
Responsibilities- Understand organizational & market priorities, trends, and goals to develop a clinical strategy.
- Interview, hire, and supervise clinical teams to staff and develop a high‑quality clinical team with strong engagement, patient‑first culture, and talent retention.
- Engage and communicate with clinicians and care teams; conduct weekly onsite center visits, hold regular individual 1:1 meetings, clinician pod meetings, and monthly leadership meetings.
- Improve primary care clinician performance through data use and coaching.
- Participate in market performance meetings on key topics such as Medical Risk Adjustment, budgeting, staffing, operational excellence, and clinical initiatives.
- Champion senior‑focused primary care strategic vision, foster stakeholder relationships with health plan partners, healthcare providers, and social services.
- Represent Center Well/Conviva brands in local communities and media activities; collaborate with recruitment to build a pipeline of high‑quality primary care clinicians (physicians, APPs, MAs, and other clinical professionals).
- Foster a patient‑centered, value‑based clinical vision, strategy, and culture locally that orients care teams around excellence in patient care, teamwork, and outcomes.
- Deliver leading clinical performance in patient experience, quality of care, clinical outcomes, and avoidable utilization.
- Periodically review clinician charts to identify opportunities and provide coaching.
- Identify critical issues for high‑risk patients and conduct root‑cause analysis with clinical and operational colleagues.
- Ensure clinicians effectively co‑manage high‑risk episodes of care with Care Integration Team resources and programs for transitions of care, high‑risk patient management, and social determinants of health.
- Monitor and manage daily patient care and initiatives to improve team‑based KPIs such as NPS, HEDIS, and Medicare quality metrics.
- Personally deliver high‑quality primary care, age‑friendly care, disease management, specialist and hospital co‑management, and complex care management.
- Spend 20‑30% of time on direct patient care; remaining time for administrative responsibilities.
- Collaborate with operational leaders and shared services partners to align on clinical and operational goals, strategic planning, budgeting, and performance metrics.
- Maintain regular communication to align on performance, strategies, and team communication, ensuring unified decision‑making.
- Work toward common goals supporting the mission, vision, values, and overall patient experience outcomes; manage clinic/market dynamics and engagement; partner on strategic and operational insights, including capital and operational budgeting.
- Review clinician schedules and incentive plans to align with patient access and management goals.
- Graduate of an accredited medical school (MD/DO) with current medical license and maintaining licensure requirements in state of jurisdiction.
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Minimum five years of outpatient practice experience, with clinical experience in primary care, senior health, and value‑based care.
- Minimum three years of experience successfully leading clinicians, demonstrated impact on talent, culture, performance, and partnership with operations.
- Skilled in clinical technology platforms and resources such as EMR systems (e.g., Athenahealth, Epic, eClinical
Works) and clinical evidence & pathway resources (e.g., Up To Date ). - Patient‑facing and part of Center Well’s TB screening program; will be screened for TB.
- Required to adhere to Humana’s flu vaccine policy.
