Base pay range
$55.00/hr - $60.00/hr
Home Health RN – 13‑week Assignment in Kāneʻohe, HawaiiCastle Home Care Services is looking for an experienced Home Health RN for a 13‑week assignment in Kāneʻohe, Hawaii. The role supports patient care in the community, working closely with families, physicians, and an interdisciplinary team to ensure continuity, quality, and timely care.
Start Date: Feb 3, 2026
Schedule: Day shift, 40 hours guaranteed weekly
Responsibilities- Coordinate patient care plans and resources
- Monitor clinical appropriateness, diagnostics, treatments, and ongoing needs
- Serve as liaison between care team, patients, and families
- Collaborate with acute care case managers on transitions of care
- Review utilization, discharge planning, and regulatory compliance
- Work with complex populations including frequent utilizers and chronic care patients
- Manage transfers and ensure continuity of information
- Document and report using Homecare Home Base and OASIS
- 2+ years RN experience
- Experience with Homecare Home Base + OASIS
- Bachelor’s degree in Nursing
Territories are assigned close to start date based on current needs. Mileage reimbursement available.
