Job Purpose:
Under the supervision of the Chief Operating Officer and Behavioral Health Director, the Behavioral Health Hybrid Therapist works as part of the Community Health Center primary care team, in both clinic and school-based settings. The role involves identifying, triaging, and managing patients with behavioral health issues. This is a non-supervisory position.
Qualifications:
Requires graduation from an accredited college with a Master’s Degree in Clinical Professional Counseling, Social Work, or Marriage and Family Therapy, and licensure in Montana as a Clinical Professional Counselor (PCLC or LCPC), Clinical Social Worker (SWLC or LCSW), Marriage and Family Therapist (IMFT or LMFT), or equivalent in a recognized behavioral health profession in Montana.
Preference given to candidates with at least two years of clinical or mental health experience or to fully licensed therapists.
Knowledge & Skills:
- Behavioral medicine and evidence-based treatments for integrated medical and mental health conditions
- Psycho-pharmacology
- Screening and diagnostic tools
- Substance abuse counseling and treatment planning
- Interviewing, screening, assessing, and counseling techniques for mental health disorders
- Mental health crisis identification, diagnosis, and management
- Client confidentiality, ethical, and legal requirements
- Community resources and organizations
Abilities:
- Assess clients and develop treatment plans or referrals
- Provide counseling and case management in clinic and school settings
- Work effectively in a team, collaborating with patients, providers, education staff, and support personnel
- Maintain confidentiality of all health center matters
- Establish and maintain effective relationships in a culturally diverse environment
- Work effectively with diverse populations and age groups
- Travel to different sites (office and/or school) may be required
Working Environment:
Work is performed in office/clinic and school-based settings, involving computer/phone work and moderate noise. Physical activities include standing, bending, sitting, and lifting. Travel to various clinic locations may be required.
Main Duties and Responsibilities:
- Provide services to patients from preadolescent to adult
- Diagnose and treat individuals with mental disorders and psychosocial issues
- Assess social needs through individual or group sessions
- Assist primary care providers in evaluating medical non-compliance and supporting patients with chronic pain
- Collaborate with the primary care team to treat and manage patients with emotional and health issues
- Refer patients to specialists as appropriate
- Detect at-risk patients and develop preventive plans
- Evaluate patient care plans with the primary care team
- Participate in meetings and quality improvement activities
- Provide staff education and training
- Travel to various Greater Valley Health Center sites to see patients as needed
