What

you will be doing

The MDS Nurse Case Manager is responsible for coordinating MDS (Minimum Data Set) assessments and discharge planning for residents. This role ensures compliance with regulatory guidelines, collaborates with the interdisciplinary team, and supports residents and families throughout the care process, from admission to discharge.

  • Conduct and coordinate the MDS/RAI (Resident Assessment Instrument) Assessment Process for assigned residents, ensuring compliance with guidelines and timely certification of MDS completions.
  • Review and update comprehensive nursing care plans to ensure the individualized needs of residents are met.
  • Schedule and conduct Care Conferences for assigned residents, working collaboratively with residents, families, and the care team to develop personalized discharge plans.
  • Review residents’ admission details and ensure the physician’s orders align with each resident’s care plan.
  • Facilitate meetings between insurance companies, skilled nursing facilities, residents, and families to ensure smooth transitions of care.

Education

Nursing Degree (Associate or Bachelor’s) required.

About you
  • Active RN or LPN licensure in the State of Michigan.
  • RAC-CT certification, preferred.
Experience
  • Proficiency with the MDS process is required.
