Hospice RN Case Manager

• Conduct ongoing assessments of the patient’s physical and psycho-social status
• Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan
• Provide education and interventions to manage pain and other physical symptoms
• Ensure availability of necessary equipment, supplies, and medications
• Revise the IDG plan of care as the patient’s needs evolve
• Document patient/family services accurately and in a timely manner
• Participate regularly in IDG meetings

• Active, unrestricted RN/LPN license in the State of Michigan
• At least one (1) year of relevant experience as an RN or LPN (required)
• One (1) year of management experience (preferred)
• One (1) year of home health experience (preferred)
• Current CPR Certification
• Experience in community health and a wide range of Medical-Surgical settings (preferred)
• Reliable transportation with a valid state driver's license and current car insurance
