- Conduct ongoing assessments of the patient’s physical and psycho-social status
- Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan
- Provide education and interventions to manage pain and other physical symptoms
- Ensure availability of necessary equipment, supplies, and medications
- Revise the IDG plan of care as the patient’s needs evolve
- Document patient/family services accurately and in a timely manner
- Participate regularly in IDG meetings
- Active, unrestricted RN/LPN license in the State of Michigan
- At least one (1) year of relevant experience as an RN or LPN (required)
- One (1) year of management experience (preferred)
- One (1) year of home health experience (preferred)
- Current CPR Certification
- Experience in community health and a wide range of Medical-Surgical settings (preferred)
- Reliable transportation with a valid state driver's license and current car insurance
