Schedule: Flexible 7 on/7 off on-call shifts (weekdays after business hours and weekends)

Position OverviewAs a Hospice Registered Nurse (RN), you will deliver exceptional end-of-life care, combining medical expertise with emotional support . You’ll be a vital part of an interdisciplinary team, dedicated to enhancing the quality of life for patients during this important phase.

- Provide compassionate, home-based care to hospice patients with a prognosis of six months or less to live.
- Collaborate with an interdisciplinary team (physician, social worker, chaplain, CNA, HHA, volunteers) to develop and execute individualized care plans.
- Conduct thorough patient assessments, monitor vitals, evaluate breathing, and manage pain using prescribed medications and treatments.
- Ensure the availability of necessary medical equipment, oxygen, and medications.
- Educate patients and families about care plans, medications, hospice goals, and what to expect.
- Offer holistic care that addresses the physical, emotional, and spiritual needs of patients and their families.
- Active, unrestricted RN license in the State of Michigan.
- Minimum of one (1) year of nursing experience (hospice or home health experience preferred).
- Experience in community health or Medical-Surgical settings (a plus).
- Reliable transportation, a valid driver’s license, and current car insurance.
