Join Our Team as a Hospice Registered Nurse (RN) – 7 On/7 Off On-Call

Location: Grand Rapids

Schedule: Flexible 7 on/7 off on-call shifts (weekdays after business hours and weekends)

Position Overview

As a Hospice Registered Nurse (RN), you will deliver exceptional end-of-life care, combining medical expertise with emotional support . You’ll be a vital part of an interdisciplinary team, dedicated to enhancing the quality of life for patients during this important phase.

Key Responsibilities
  • Provide compassionate, home-based care to hospice patients with a prognosis of six months or less to live.
  • Collaborate with an interdisciplinary team (physician, social worker, chaplain, CNA, HHA, volunteers) to develop and execute individualized care plans.
  • Conduct thorough patient assessments, monitor vitals, evaluate breathing, and manage pain using prescribed medications and treatments.
  • Ensure the availability of necessary medical equipment, oxygen, and medications.
  • Educate patients and families about care plans, medications, hospice goals, and what to expect.
  • Offer holistic care that addresses the physical, emotional, and spiritual needs of patients and their families.
Qualifications
  • Active, unrestricted RN license in the State of Michigan.
  • Minimum of one (1) year of nursing experience (hospice or home health experience preferred).
  • Experience in community health or Medical-Surgical settings (a plus).
  • Reliable transportation, a valid driver’s license, and current car insurance.
