The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services so older individuals can continue living in the community.

Provide care services to participants including assistance with shopping, running errands, laundry, housekeeping and meal preparation within their home, community setting and/or day center as directed by your leader or designee in accordance with the participant assessment and plan of care.
Key Responsibilities- Provides ADLs (shopping, laundry, meal preparation, housekeeping, medication reminders, etc.) in the participant homes that are essential for their health and independence.
- Observe, record in medical record and report changes in participant emotional, behavioral and physical condition to appropriate disciplines and/or leader.
- Encourages each participant autonomy and dignity by communicating in a respectful and sensitive manner according to their needs.
- Promotes participants to engage at maximum level of independence during care.
- Must care for, support and collaborate with everyone involved in the participant care including staff and the participant family using the participant-centered care model.
- Must possess, as a minimum, a high school diploma, or equivalent.
- Well-developed ability to show discretion and maintain confidentiality.
- Good communication skills, both written and verbal.
- Detail oriented with good problem-solving skills.
- Basic computer skills.
- Minimum of one year of experience working with the frail, elderly, or long-term care population.
- Communicates with staff, participants, caregivers, and other organizations as necessary.
- Works beyond normal working hours and on weekends and holidays when necessary.
- Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
- May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
- Required to drive a car in all types of weather.
- Provide care and support for a diverse population of individuals, including those who are ill, disabled, elderly, and may be experiencing behaviors associated with dementia and cognitive limitations within the Senior Care Partners PACE Program and/or center.
- Must be able to cope with the mental and emotional stress of the position.
- Willingness to work in various home environments which may include dust, animals, pet dander, temperature changes, smoke, pests, strong odors, and variations in cleanliness.
- Must have the physical ability to perform duties such as shopping, laundry, housekeeping, meal preparation, bathing, dressing, toileting, and medication reminders.
- Lifting up to 50 lbs., pushing and/or pulling up to 20 lbs. with force, prolonged periods of bending, squatting, twisting, reaching, standing, stairs, and other physical activity.
- Must function independently, have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.
- May be necessary to assist in the evacuation of participants during emergency and/or disaster situations.
- Must have reliable transportation, current valid Michigan driver’s license and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases:
Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis
B. - Must be willing to work beyond normal hours and be part of our on-call rotation.
