Position SummaryThe Medical Social Worker provides comprehensive social work, care management, and behavioral health support to pediatric patients and families within Bronson Pediatric Gastroenterology Specialists. This role may support patients across both hospital-based and ambulatory practice settings, depending on assignment. The Medical Social Worker collaborates closely with physicians, interdisciplinary care teams, and community agencies to address psychosocial needs, coordinate care, support crisis intervention, and promote continuity of care.
Key ResponsibilitiesClinical Social Work & Care Management
- Assess psychosocial, emotional, behavioral, and environmental needs of patients and families
- Provide crisis intervention related to mental health, suicide risk, abuse/neglect, grief, domestic violence, substance use, and acute stress
- Administer behavioral health screenings and coordinate further assessment when indicated
- Deliver brief, focused interventions to support coping, behavior change, and emotional wellbeing
- Advocate for patients and families to address barriers to care
- Collaborate with physicians and interdisciplinary teams to develop individualized plans of care
- Coordinate referrals to community resources, behavioral health providers, and specialty services
- Support timely and safe discharge planning and transitions of care
Ambulatory Practice-Based Social Work
- Provide care management and behavioral health services in the outpatient practice setting
- Identify high-risk psychosocial needs and provide early intervention
- Respond to crisis situations within the ambulatory setting
- Support patients with mild to moderate behavioral health concerns
- Promote culturally responsive, patient- and family-centered care
- Coordinate ongoing support services and follow-up care
Documentation & Compliance
- Thoroughly document assessments, interventions, referrals, and care plans in the electronic health record
- Maintain accurate, timely, and compliant documentation
- Participate in departmental policy and protocol development
- Uphold strict confidentiality and professional standards
Collaboration, Education & Professional Development
- Work collaboratively with interdisciplinary healthcare teams
- Provide education to patients, families, and healthcare team members
- Mentor and supervise students as assigned
- Participate in committees, quality improvement initiatives, and professional development activities
- Foster a positive, collaborative work environment
Hospital Medical Social Worker
- Master’s degree in Social Work with LMSW or LLMSW in good standing
- OR Master’s degree in Psychology or Counseling with LPC, LLPC, LP, or LLP licensure
- Fully licensed within two years of hire, including required supervision hours and licensing exam
- 1–2 years of professional experience in healthcare or mental health preferred
Practice Medical Social Worker
- Master’s degree in Social Work with one year of recent medical or mental health experience
- Limited license candidates considered with required experience
- OR Master’s degree with LLPC or LPC licensure
- Fully licensed within two years of hire
- Valid Michigan driver’s license and proof of insurance required for Bronson at Home responsibilities
- Strong verbal and written communication skills
- Excellent interpersonal, advocacy, problem-solving, and critical thinking skills
- Ability to work effectively with patients, families, physicians, and community partners
- Customer service-oriented and culturally responsive approach
Physical Requirements
- Frequent movement throughout hospital and clinic environments
- Sustained mental and visual focus
- Regular lifting up to 50 pounds and occasional assistance with heavier tasks
