* Job Description:

** - Join a dynamic and dedicated team focused on improving patient outcomes! - Actively manage patient care and collaborate with medical providers to establish personalized healthcare goals.

- Coordinate multi-disciplinary care conferences for high-risk or complex patients.

- Continuously review electronic health records to manage acute and chronic care needs.

- Advocate for patients and families, fostering independence and self-care.

- Efficiently address patient needs with problem-solving and anticipatory skills.

- Facilitate patient referrals within and outside the facility, ensuring timely care.

- Perform nursing assessments, documentation, education, and medication administration.

- Maintain annual competencies and a clean, safe working environment.

- Build rapport with interdisciplinary teams to enhance collaboration and efficiency.

- Participate in continuous improvement efforts in case management.

- Engage in triage and emergency response as needed.

- Take lead responsibilities in specialty clinics, including managing referrals and outpatient services, tracking progress, and supporting educational initiatives.

- Be the primary case manager for obstetric patients and oversee postpartum care initiatives.

- Communicate effectively with specialist providers through video teleconferencing.
** Education, Certifications, and Licenses

Required:

** - Graduate of an accredited school of nursing (Bachelor’s degree preferred).

- Current, unrestricted Registered Nursing license in Alaska.

- Basic Life Support certification preferred.

- National Case Management certification preferred.

- High school diploma or equivalent required.
** Experience

Required:

** - Minimum of 2 years of nursing experience, including case management or at least 1 year in an ambulatory setting.

- Familiarity with SEARHC services and other tribal health organizations in Alaska.

- Strong customer service principles and safety protocols.

- Technical proficiency with computers and medical equipment.

- Excellent communication skills, both oral and written.
** Abilities:** - Multi-tasking and independent work in a fast-paced environment.

- Problem-solving and conflict resolution to maintain effective working relationships.

- Ability to follow directions and perform multiple tasks simultaneously.

- Capacity for mobility and physical stamina in a clinical setting.
** Shift Information:** - Monday to Friday, 8:00 am - 5:00 pm
