Job Description
Summary:
Minimum starting salary $22.20 / hour. The Licensed Practical Nurse is a licensed nursing care provider who is responsible for delivering a full range of support services to individuals impacted by behavioral health issues (addiction/substance use and mental health). The LPN is responsible for facilitating the maximum level of independent functioning of the clients served and demonstrates a warm, empathic, hopeful attitude, conveying a philosophy of dual recovery.
The LPN is trained and/or experienced in working with severe mental illness and/or substance abusing diagnosed individuals to assess, plan, develop, coordinate, and provide support services. The LPN provides services to individuals impacted by behavioral health issues, recognizing that individuals we serve often have experienced trauma that affects their development and adjustment. The LPN will be trained in trauma-informed care and will work to provide services in an environment that is sensitive and responsive to prevent victimization, abuse, or trauma.
The LPN will report to the Nurse Coordinator of Adult Psychiatric Practices.
- High School diploma or equivalent
- Diploma in Practical Nursing
- Experience working with behavioral health and substance use disorders
- The LPN meets the total clinical needs of the practice patients as determined by the Psychiatrist or Advanced Practice Nurse in consultation with the psychiatrist/physician or as ordered.
- The LPN reports to the Nurse Coordinator of Adult Psychiatric Practices and may report to the Physician or APN as appropriate.
- The LPN performs duties as assigned by the Nurse Coordinator of Adult Psychiatric Practices or the physician/APN.
- The LPN communicates with higher level nursing and/or physician, maintains appropriate medications and logs as required through the clinic.
- The LPN facilitates efficient patient flow during office visits, identifies patient problems, communicates pertinent data to the physician/provider, and assists with chart/charge preparation and data collection.
- The LPN makes appropriate referrals, uses standing orders properly, and ensures documentation in electronic health records.
- The LPN actively engages patients and family and assists in educating the patient’s plan of care.
- The LPN meets the productivity standards of the mental health office.
- Perform all other duties as assigned.
