The Assertive Community Treatment (ACT) Registered Nurse (RN) provides nursing services to adult clients enrolled in the ACT program that are interested in improving their health and wellness. RN assesses physical and psychiatric health strengths, needs, and interests as it applies to psychiatric nurse management, makes appropriate referrals to community physicians, provides management and administration of medication in conjunction with the ACT psychiatric provider, provides information and education, adds health promotion goals/objectives/interventions to treatment plans, and accurately documents progress via progress notes.
RN provides cross-training and consultation to the treatment team about nursing services and health promotion as interventions for any patient interested in health promotion and intervention. RN demonstrates a warm and welcoming, empathic, hopeful attitude, with a strengths-based focus on recovery. RN will be trained in trauma-informed care and will work to help provide services in an environment that is sensitive and responsive to patient needs and goals.
RN reports directly to ACT/ITCD Programs Supervisor.
- Requires a minimum of bachelor’s degree in nursing
- Must be licensed as an RN in the state of Missouri.
- Demonstrate time management and multitasking skills to independently manage team caseload. Arrange work schedule to see clients regularly according to their individual needs. Manage multiple, simultaneous deadlines to ensure that services are provided according to established expectations. Manage pharmaceutical and medical supplies.
- Collaborate with each patient to develop individualized person-centered plans to guide treatment. Plans are to be completed within established time frames and reviewed/updated bi-annually anize and manage the system of getting medication to clients and integrating medication administration into patient’s individualized person-centered plans. Provide medication education to clients, provide symptom education to enable clients to identify their behavioral health symptoms, participate in treatment, rehabilitation, and support services.
- Effectively assess and monitor clients’ participation and progress in treatment according to and in adherence with individualized person-centered plans. Provide or assist clients with transportation to/from appointments. Maintain contact with clients’ families/guardians/natural supports as appropriate and approved by patient.
- Ensure that required assessments are completed at intake and quarterly, at minimum, and in collaboration with each patient whenever possible.
- Provide treatment interventions which are medically necessary, ethical and in accordance with patient’s individualized person-centered plans. Help the treatment team consider nursing services and health promotion as treatment interventions for helping clients gain maximum independence and satisfaction.
- Communicate all relevant clinical information with ACT/ITCD Programs Supervisor and other key ACT team members regarding each patient’s critical care needs in a timely manner.
- Ensure that all required progress notes, nursing assessment, AIMS, and all other documentation is completed within established time frames and according to quality standards set by State and Federal regulations, CARF certifications, program expectations, ACT Fidelity, State and Federal regulations, as well as Ozark Center Policy and Procedure.
- Support coworkers and teammates in providing trauma-informed, person-centered services to clients. Collaborate effectively with colleagues, service providers and treatment team members to support clients in achieving their recovery goals.
- Proactively adopt and implement a variety of new and ongoing program initiatives to benefit clients served. Network with community members, potential referral sources, medical providers, psychiatric units, and other community resources to benefit clients served.
