• Oversee discharge planning process, utilizing the strength of both Case Management and Social Services to ensure the appropriate resources and services are available and arranged for patients.
• Communicate with third party payor case management/insurance companies and provide documentation/communication as required.
• Develop and maintain a marketing network with professionals in the community.
