Overview
Discharge Planner role at Saint Joseph Medical Center – Joliet. Department:
Case Management/Social Services. Full‑time, days Monday‑Friday. Prime Healthcare Saint Joseph Medical Center, Joliet, IL 60435.
Coordinate activities of the Case Management Department—utilization management, social services, performance improvement. Develop discharge plans, assist with case management referrals and authorizations. Maintain and coordinate Medi‑Cal (TARS) presentation to on‑site reviewer and file accordingly. Perform discharge planning duties: interview patients/families, gather data to develop safe discharge plans.
QualificationsKnowledge of discharge planning, utilization management, and case management terminology in managed and non‑managed care environments. Basic to intermediate computer skills. Current BCLS (AHA) certificate required upon hire and maintain current. RN license preferred. LVN license preferred with 5 years experience.
