Overview
Under the direction and immediate supervision of the Network Director, the Team Leader/Qualified Mental Health Practitioner (QMHP) performs clinical duties, such as being responsible for the overall coordination, supervision, and fidelity of the FIRST.IL program, ensuring adherence to the Coordinated Specialty Care (CSC) model and maintaining communication with DHS/DBHR staff. This position oversees all team functions, conducts intake assessments, engages individuals and families in services, and facilitates smooth transitions from hospital discharge or crisis stabilization to outpatient care.
Salary: $62,000-$65,000
Shift: Monday-Friday, 3 weekday and 2 evenings

- Provide oversight of team operations, including clinical services, outreach, referrals, and program workflow.
- Conduct intake assessments and engage individuals and families in services.
- Facilitate timely and coordinated transitions from hospital discharge or crisis stabilization to outpatient care.
- Lead and facilitate weekly multidisciplinary team meetings.
- Guide clinical interventions and ensure evidence-based practices are implemented
- Provide weekly supervision to team members
- Coordinate CBT-p consultations and participate in statewide learning collaboratives.
- Ensure all staff complete required trainings within 30 business days of hire.
- Implement and oversee Family Psychoeducation (FPE) as a core program component.
- Engage families in treatment planning and recovery support
- Deliver family education, including education on psychotic disorders, coping strategies, and effective communication.
- Integrate family discussions into team meetings and serve as a liaison between the recovery team and family system.
- Foster collaboration, reduce stigma, and promote recovery through shared decision-making.
- Participates in regular leadership team meetings
- Maintains service records, database, and statistics.
- Demonstrated knowledge of correct procedures for Medicaid billing
- Completes intakes, verifies insurance, and manages billing.
- Monitor programmatic goals and outcomes continually adapting to meet or exceed them.
- Ensure that all documentation and communications align with Trinity Services’ mission, values, and operational needs.
- Completes daily paperwork and data collection notes in a detailed, accurate, consistent and timely manner
- Handle all confidential information in a professional and secure manner
- Other duties as assigned
- Considerable knowledge of the techniques and principles of child and family development, family systems theory, individual, family, behavioral therapy (ACT, DBT), and crisis focused interventions.
- Strong communication, leadership, and counseling skills.
- Ability to independently diagnose using the DSM 5.
- Strong command of treating people with chronic mental illness.
- Remain in a stationary position, often standing or sitting for prolonged periods
- While performing the duties of this job, the employee is regularly required to stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard; reach, stoop, kneel to install computer equipment
- Physical activities that could be used include: pulling, pushing, standing, grasping, stooping, crouching, reaching, lifting, and repetitive motion.
- Completion of Master’s Degree in psychology, counseling, social work or related field. Clinical License (LCPC, LCSW, LMFT) preferred
- Completion of Quality Behavior Solutions (QBS) Safety Care training, CPR, and First aid (upon hire).
