Job Summary

Under the direction of the Director of Pharmacy, the Staff Pharmacist

oversees the day-to-day workload of the department to provide high quality pharmaceutical services for the hospital patients and employees. This includes supervising and verification of all technician-handled orders, performing

clinical duties, calculations as required and communication as needed with

physician and all hospital personnel and patients.

II. Duties And Responsibilities

l. Computer order entry and filling of orders, including any necessary activities to obtain medications, clarify orders and solve any problems related to providing the proper medication to the proper patient as ordered by the physician.

  • Maintenance of Omnicell functions, reports, etc.
  • Calculation of medication doses/schedules with documentation
on patient medical records to assure proper medication is

provided to the nursing staff for administration to the patient

as ordered by the physician.
  • Monitoring and dosing of aminoglycosides and Vancomycin, Performing DVT assessments, assessing all CHF, pneumonia,
and renal patients for appropriateness of drugs, ADR, Med

errors, and patients on Glucophage. Review of daily C&S

lab results for antibiotic appropriateness . The pharmacist is also

responsible for nutritional assessments/ recommendations

on patients on TPN therapy when requested by the physician.

The clinical pharmacist will make weekly rounds with a

multidisciplinary team to assess patient needs.
  • Checking medication carts, IV piggybacks and large volume fluids
  • Providing medication information for physicians, nurses, and
hospital employees as requested.
  • Contacting physician whenever needed to clarify physician
orders, i.e. proper dosage, route of a::lministration, or allergy.
  • Monitoring and coordinating technician duties as needed
  • Covering all "on call" responsibilities including responding
to cardiac and trauma codes, assisting the house supervisor

as needed for orders written during hours the pharmacy is

closed. This includes coming back into the hospital for

orders written after hours if the medication is not available

in Omni cells and cannot wait until the pharmacy reopens.
  • Documenting narcotic purchases and disposition throughout Wayne Memorial Hospital. Operating Secure Vault and
maintaining and retrieve reports.
  • Ordering medications from wholesale and direct vendors
  • Checking an approved list of medications each day to
make suggestions to the physician to change from IV to

PO dosing for cost savings.
  • Medication counseling on any hospital-wide approved pathway
  • ADR and Med Error follow ups following nursing reports
Other Responsibilities

1 . Inventory control purchasing and returns
  • Prepare Epidural Infusions
  • Oversee completion of monthly floor checks.
  • Committee assignments/ Quality Assurance record keeping as assigned
III. Education

B.S./ PHARM D in Pharmacy

IV. Qualifications And Experience

Current Ga. Pharmacist License or eligible for Ga. License.

High level of supervisory skills, organizational skills, communication skills, reliability and attention to detail. Previous hospital training preferred.

Previous pharmacokinetic training required.

V. Physical Demands
  • Carry less than 10 pounds on a regular basis and up to 3-5 pounds if needed.
  • Ability to conduct training programs and presentations for hospital patients and employees as needed.
  • Ability to properly write physician's orders concerning clinically and
pharmacokinetically calculated doses of medication.

VI. Responsibility For Supervision

Supervises six non-exempt employees.

Internal
