About the Role

The RN Clinical Coordinator is a critical front‑line clinical role responsible for assessing patient concerns and guiding them to the appropriate level of care through virtual platforms. Serving as a primary access point for patients, the RN Clinical Coordinator ensures safe, timely, and patient‑centered triage while supporting continuity of care across the Dimer Health clinical team.

This role blends strong clinical judgment with excellent communication and care coordination skills in a fully remote, virtual‑first environment.

  • Provide clear patient education, self‑care guidance, and follow‑up instructions
  • Communicate calmly and empathetically with patients experiencing acute, chronic, or new symptoms or concerns
  • Support medication refill requests and address other clinical inbox matters within scope
Care Coordination & Team Collaboration
  • Serve as a clinical liaison between Administrative and Clinical Teams to support timely care delivery
  • Assist with care coordination in partnership with the Case Manager to support continuity of care
  • Support timely scheduling and patient “conversion” to appropriate visits when indicated
Clinical Inbox, Documentation & Operations
  • Review and respond to patient messages within defined service‑level expectations
  • Prepare and distribute work notes, school notes, and other post‑visit documentation as appropriate
  • Document all patient interactions accurately and thoroughly in the electronic medical record (EMR)
  • Perform administrative and operational support tasks as needed to ensure efficient care delivery
Compliance & Privacy
  • Maintain patient privacy and confidentiality in accordance with HIPAA and telehealth regulations
  • Adhere to clinical, operational, and quality standards in a remote care environment
