HRC Case Manager Position

Join Hyacinth Foundation as an HRC Case Manager to support clients living with HIV in Jersey City, NJ.

Full‑time

• Provide Harm Reduction specific community case management and referral services.
• Conduct community outreach, provider education, and client recruitment of new and out‑of‑care clients.
• Deliver education presentations about Harm Reduction to providers throughout Hudson County.
• Develop individualized client assessments and service plans.
• Refer clients to medical care, substance use treatment, and other supportive service needs.
• Maintain client case records, establish and update client case records, and prepare monthly reports.
• Represent Hyacinth at community events, meetings, and conferences.
• Attend required opioid and harm reduction meetings and other trainings.

• 1–3 years of experience in social services or community services.
• Bachelor’s Degree in a related field.
• Active listening skills and familiarity with trauma‑informed care.
• Knowledge of available services and community resources in North Hudson.
• Bi‑lingual (English and Spanish) preferred.

• Experience working in Hudson County communities.
• Knowledge of supportive services and community resources.
• Experience with grant reporting and data collection.
• Ability to work non‑traditional hours, including late nights and weekends.

• Excellent oral presentation and writing skills.
