The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Job OverviewWill be working in the community with consumers with severe and persistent mental illness in assisting them with individual service coordination, training in daily living skills, residential counseling (if applicable), support services, crisis intervention services, education and facilitating proper medication self-administration techniques (if applicable), and health care monitoring and oversight services.
QualificationsRequired:
- Must have a BSW/BA in Social Work, Psychotherapy, or related field
- A minimum of 1 year experience in mental health related field with adolescent/adult population
Certification/Licensure
- Must have a current driver's license issued in the driver's state of residence without restrictions and with all necessary endorsements appropriate to the class of vehicle being operated
- Crisis Intervention Training
- BLS / CPR Certified through American Heart Association
- Must be computer literate
- Excellent communication and interpersonal skills

Ability to be a Member of Interdisciplinary Team:
- Participates in case conferences, staff meetings, individual supervisions and case presentations, and onsite/off-site trainings as needed.
- Serves as a broker between the needs and perspective of the consumer and the goals of the treatment system; may include educating treatment providers and advocating on behalf of the consumer.
- Works cooperatively and respectfully while demonstrating the ability to compromise with co-workers.
- Effectively communicates case information to all team members as indicated.
- Understands role in interdisciplinary team.
- Regularly seeks timely and appropriate assistance from management, and other resources.
- Acts as a model for professional behavior.
Documentation
- Completes Individualized Rehabilitation Plans with consumers in a timely basis, per program’s policies and procedures and consults with treatment team regarding initial and ongoing assessments.
- Demonstrates the ability to clearly write comprehensive progress notes; progress notes that reflect the wealth of information obtained, assessment and plan within prescribed time frames and as per program’s policies and procedures.
- Keeps accurate and updated to date record of information/documentation in consumer’s charts.

- Establishes and maintains supportive and therapeutic relationship with consumers.
- Exhibits a positive and professional demeanor with his/her/their consumers, characterized by empathy, compassion, warmth, calmness and respect.
- Demonstrates the ability to use a wide range of verbal interventions, consistent with the model: clarify, reflect, redirect, interpret, reframe, confront, etc.; with appropriate timing and clinical justification.
- Demonstrates the ability to assist consumers in understanding and coping with stressors that trigger potential deterioration.
- Able to verbalize knowledge as to when a change in the level of care is warranted.
- Demonstrates proficiency in teaching consumers how to improve daily functioning in order to improve treatment outcomes, this includes but is not limited to educating consumers on activities of daily living, medication, social skills, financial budgeting, etc.
