• Deliver home-based care to patients recovering from illnesses, injuries, and chronic diseases under RN supervision.
• Collaborate with a dedicated team of physicians, RNs, CNAs, and aides to ensure quality patient care.
• Monitor patient vitals, conduct assessments, document progress, and report changes to your RN supervisor.
• Assist RNs in implementing patient care plans and teaching self-care techniques.
• Educate families on care plans, medications, and home safety practices.
• Work with an interdisciplinary team to coordinate patient care and discharge planning.

• Active, unrestricted LPN license in the State of Michigan.
• At least one (1) year of clinical experience as an LPN.
• Current CPR Certification.
• Reliable transportation, valid state driver's license, and current car insurance.
