• Deliver home-based care to patients recovering from illnesses, injuries, and chronic diseases under RN supervision
• Collaborate with a dedicated team of physicians, RNs, CNAs, and aides to ensure quality patient care
• Monitor patient vitals, conduct assessments, document progress, and report changes to your RN supervisor
• Assist RNs in implementing patient care plans and teaching self-care techniques
• Educate families on care plans, medications, and home safety
• Work with an interdisciplinary team to coordinate patient care and discharge planning

• Active, unrestricted LPN license in the State of Michigan
• At least one (1) year of clinical experience as a Licensed Practical Nurse
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance
