Hiring an RN Case Manager to support a new PACE center providing coordinated care for active elderly participants. This role focuses on in-home skilled nursing visits, clinical assessments, and care plan management in collaboration with providers and an interdisciplinary team.
Key Duties:- Perform initial, annual, and semi-annual assessments
- Conduct skilled nursing visits (vitals, meds, IVs, wound care, labs, blood sugar checks)
- Develop and implement care plans with providers and IDT
- Complete home safety evaluations and participant education
- Supervise personal care assistants and ensure quality care
- Schedule:
Monday–Friday, 8:00 AM–5:00 PM - This role will include home visits to patients home within 10-20 miles of Jeffersonville
- RN license (BSN preferred)
- 1+ year experience with elderly populations (home care, LTC, hospice, etc.)
- BLS, valid driver’s license, reliable transportation
