Responsibilities
Perform comprehensive assessments of high-risk patients to determine clinical, functional, and psychosocial needs
Collaborate with primary care providers to develop and implement individualized care plans with targeted interventions
Continuously reassess patient status and adjust care plans to ensure appropriate, cost-effective levels of care
Coordinate care transitions across settings, including hospital discharges and community-based services
Identify and arrange non-medical supports such as transportation or housing to promote adherence to treatment plans
Required Qualifications
License
Required:
RN license in Indiana (compact license accepted)
Preferred
Skills:
Wound care, IV therapy, drain care, G-tubes, and basic patient education
Home Care Home Base (HCHB) experience preferred
Ability to analyze complex medical information and make rational clinical decisions
Case Management certification
BLS/CPR
