CMT/CNA FT Evenings 3p-11p at Stone Bridge Senior Living
Our care team plays an essential part in the daily life of our residents. As a Certified Medication Technician (CMT) and Certified Nursing Assistant (CNA), you will assist in the administration of medications and support medical staff in providing comprehensive care to residents with various injuries, illnesses, or disabilities.

- Follow the policies and procedures of the facility governing the administering of medications to residents.
- Note and report errors in the administration of medications.
- Assist in developing and implementing procedures or programs of the facility that seek to determine what medication errors are taking place, when, where, and why they occur, and how they can be prevented.
- Perform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be required.
- Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you.
- Accurately measure, record, and report the vital signs of residents.
- Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents.
- Follow the medication card practices of the facility.
- Follow the facility procedure for recording the order in the resident’s clinical record, including the type, route, dosage, frequency and strength of each medication.
- Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
- Assists resident with or performs activities of daily living for residents in accordance with care plans and established policies and procedures.
- Assists residents with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
- Coordinates dining room services at assigned mealtimes, including set‑up and clean‑up, meal tray delivery, feeding assistance, and documentation of meal intake.
- Delivers nutritional supplements to residents at assigned times and provides assistance as necessary to ensure intake. Documents intake accordingly.
- Assists with weighing residents according to facility policy, and records weight in designated locations. Reports weight changes to nurse and supervisor.
- Assists with tracking the condition of the resident’s skin. Reports any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor.
- Assists nursing staff in carrying out toileting program activities.
- Completes flow sheets daily to indicate that the specified task was done. Documents the progress of each resident as directed by supervisor.
- Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e., resident refusal, reassigned to nursing unit).
- Reports change in a resident’s condition to the resident’s nurse and supervisor.
- Must possess, as a minimum, a high school education or its equivalent.
- Must be a licensed medication tech having successfully completed a state‑approved training program and any necessary examination(s) in accordance with laws of this state.
- Excellent communication and interpersonal skills.
- Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
