Jefferson City Nursing and Rehabilitation Center is currently hiring a full‑time MDS Coordinator to oversee the Minimum Data Set assessment process for our residents. The role requires accurate and timely assessments, facilitation of comprehensive individualized care plans, and maintenance of compliance with state and federal regulations. RAC certification is required.
Responsibilities• Oversee the Minimum Data Set assessment process for residents.
• Complete accurate and timely assessments.
• Facilitate the development of individualized care plans.
• Ensure compliance with state and federal regulations.
The MDS Coordinator position requires a Registered Nurse or Licensed Practical Nurse who is RAC certified.
Schedule & Compensation• Monday‑Friday, 8:00 am – 4:30 pm.
