The Registrar Nephrology is a physician in specialty training who delivers comprehensive nephrology care exclusively through virtual platforms. The role focuses on the assessment, diagnosis, and management of acute and chronic kidney diseases using telehealth technologies, ensuring safe, evidence-based, and patient-centred care under consultant supervision, while supporting continuity of care and chronic disease management.
Virtual Patient Consultations and Clinical Management- Conduct video and phone consultations for patients with nephrology related conditions.
- Assess and manage chronic kidney disease (CKD), hypertension, electrolyte disorders, proteinuria, haematuria, nephrolithiasis, and post AKI follow up.
- Review laboratory results and imaging remotely and adjust management plans accordingly.
- Identify red flags requiring urgent in person assessment or hospital referral.
- Develop individualized treatment and monitoring plans in collaboration with consultants.
- Ensure accurate and timely documentation in the electronic medical record (EMR).
- Arrange virtual follow ups and coordinate care with primary providers.
- Monitor CKD progression using laboratory trends and patient reported outcomes.
- Provide virtual counselling on lifestyle modification, diet, fluid balance, and medication adherence.
- Support anaemia of CKD, CKD MBD, and blood pressure management remotely.
- Collaborate with dietitians and allied health professionals via telehealth platforms.
- Coordinate dialysis preparation education and timely referrals when indicated.
- Adhere to nephrology clinical guidelines, telemedicine regulations, and ethical standards.
- Ensure safe prescribing practices, including dose adjustments in renal impairment.
- Monitor and counsel patients regarding medication side effects and warning signs.
- Report patient safety concerns and adverse events to the Quality Department.
- Participate in virtual audits, quality improvement initiatives, and clinical reviews.
- Utilize telemedicine platforms for video consultations, secure messaging, and lab result reviews.
- Maintain accurate, complete, and confidential digital patient records.
- Optimize use of remote monitoring data (e.g., home blood pressure readings).
- Participate in telehealth training and collaborate with IT support for system optimization.
- Communicate diagnoses and management plans clearly and empathetically.
- Provide education on kidney disease progression, treatment options, and red flag symptoms.
- Address patient concerns and follow up questions promptly via telehealth channels.
- Encourage adherence to treatment plans and follow up schedules.
- Collaborate virtually with nephrology consultants for case discussions and escalation of care.
- Coordinate care with primary care physicians, nurses, pharmacists, and dietitians.
- Participate in multidisciplinary virtual meetings and care planning.
- Maintain up to date knowledge in nephrology.
- Attend CME, workshops, and conferences.
- Participate in case discussion and academic activity.
- Support teaching and supervision of junior doctors when required.
