Overview
Position: 0.8 CNA, 10:00 PM – 6:30 AM. Based on Sandstone or as assigned. SU-W-TH-SU-M-TU-F-SA. RH025197
CNANPSC.
Performs patient care under the direction of a registered nurse in compliance with nursing service policies and procedures.
Responsibilities- Provides care to the assigned resident group (95% of duties).
- Works as a team member to ensure residents receive necessary care and services.
- Immediately reports resident health concerns to the RN.
- Responds to call lights of all residents in a timely manner.
- Reports all allegations of abuse, neglect or misappropriation to the supervisor.
- Assists residents at meal and snack times and maintains adequate nutrition and hydration, offering thickened liquids and therapeutic diets as needed.
- Maintains resident safety during transfers and when using mechanical lifts, gait belts, etc.
- Ensures supportive devices are applied properly.
- Delivers restorative care specific to each resident and follows the functional maintenance program.
- Documents vital signs, weights, intake and output, observation flow sheets, restorative nursing and other assigned charting in a timely fashion as directed by licensed nursing supervisors.
- Assists in emergency situations (e.g., fire, tornado, disaster) as assigned.
- Performs other duties as assigned (5%).
- May be mandated to stay past the normal shift to meet minimal staffing requirements.
- Education:
Completion of a 120 hour nursing assistant course at an accredited program. - Experience:
Long-term care experience preferred. - Licensure/ Certification:
Current certification with the Wisconsin Nurse Aide Registry. - Special Requirement:

- Ability to sit, stand, bend, and stoop for periods of time.
- Ability to lift up to 50 pounds occasionally.
- Ability to work in a stressful environment and to respond to emergency/crisis situations.
- Ability to push/pull a medication cart.
- Exposure to noise, blood and/or body fluids.
- Shows respect for residents, staff and visitors by accepting assignments willingly.
- Has a customer service attitude; uses a kind and gentle approach with residents while maintaining dignity and privacy.
- Maintains prompt and regular attendance; reports to work as scheduled and adheres to shift times.
- Follows the Rock Haven dress code and organizes/ completes assignments in a timely manner.
- Carries out facility policies and completes a minimum of 12 in-service hours annually.
- Ability to communicate effectively orally and in writing; ability to present information and respond to questions from managers, employees, residents, families, professionals and the public.
- Ability to work well as a team member.
