Licensed Practical Nurse (LPN) – Full‑Time

Transitions At Home is seeking a compassionate and dependable Licensed Practical/Vocational Nurse (LPN) in the Janesville area to provide skilled and compassionate care to patients in their homes.

Base pay range: $25.00/hr - $32.00/hr

• 40 hours per week (8:00 am – 4:30 pm), Monday – Friday
• No weekends or holidays
• Limited on‑call rotation

• Perform routine visits and nursing assessments
• Manage medications and administer treatments
• Coordinate services and counsel patients and families per physician orders and care plan
• Document care and maintain accurate records under RN Clinical Manager supervision

• Active Wisconsin LPN License
• At least two (2) years of nursing experience; one (1) year of home health experience preferred
• CPR certification
• Valid Wisconsin driver’s license and car insurance
