Overview
Registered Nurse, Long-Term Care / Sub-Acute Unit
Location: Mercyhealth Hospital and Trauma Center - Janesville, WI
Shift: Evenings (2:30P - 11:00P);
Weekend and Holiday Rotation
Hours: 32 hours per week

Requirements:
- Graduate of an accredited nursing program; BSN preferred
- Current WI RN licensure by the WI DSPS
- Approved CPR/BLS certification within 90 days of hire

Responsibilities:
- Applies the nursing process and critical thinking in collaboration with interdisciplinary teams.
- Performs patient assessments, plans and implements interventions, and evaluates outcomes.
- Administers medications and treatments per policies, standards of care, and state regulations.
- Maintains accurate and comprehensive documentation.
- Develops and executes individualized care plans.
- Provides patient and family education using available resources.
- Participates in performance improvement and shared governance initiatives.
- Offers clinical guidance, resolves system issues, and escalates concerns appropriately.
- Promotes patient safety, quality care, and positive patient experiences.
- Engages in ongoing professional development and evidence-based practice initiatives.
- Participates in peer review and feedback processes.
- Ensures timely patient flow, manages phone calls, and performs telephone triage (clinic/ambulatory settings).
