Job Overview

Provides person-centered behavioral health assessment, care, resources and education to clients in need of crisis services. Provides support for emergency rooms, law enforcement, and other community agencies. Provides treatment planning, short-term treatment, and linkage to ongoing care for clients across the lifespan. Pursues the least restrictive disposition that will meet the determined safety needs of clients. Prioritizes needs of the community and demands on the crisis unit.

Reviews and oversees development and implementation of safety plans. Documents care provided. Collaborates with internal and external colleagues. Participates in clinical and administrative meetings. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and “Commitment to My Co-Workers.”

Supervision and Loan Forgiveness

Human Services will provide clinical supervision toward licensure for this position when staff have an APSW or LPC-IT, dependent on approval by the Department of Safety and Professional Services. Rock County participates in the Public Service Loan Forgiveness program.

Compensation and Pay Differentials

Human Service Professionals working in the Crisis Unit between the hours of 7:00 pm – 7:00 am, Monday through Thursday or between 7:00 pm Friday and 7:00 am Monday will be paid a night/weekend differential of $2.65 per hour for each night or weekend hour worked.

Responsibilities
  • 75% Provides mobile person-centered behavioral health care services for clients experiencing, or at risk of, a mental health or substance use disorder crisis
    • Provide support and resources to crisis line callers.
    • Assess clients in crisis to determine least restrictive disposition that will assure client safety; may include safety planning, voluntary admission to an inpatient or stabilization facility, or involuntary admission to an inpatient facility.
    • Coordinate admissions to the appropriate receiving facility.
    • Coordinate medical clearance if required with police, referring facility, and hospital ER staff.
    • Develop person-centered safety plans, crisis response plans and crisis plans when clinically indicated.
    • Coordinate client care and linkage to ongoing behavioral health services as assigned.
    • Facilitate detox facility admissions for individuals under 51.45; and provide post-discharge risk-needs assessments for clients discharging from a detox facility.
    • Provide behavioral health risk assessment at the Rock County Jail and Youth Services Center.
  • 15% Ensure program and medical record compliance
    • Follow all departmental as well as divisional policies and procedures.
    • Document client care in accordance with policies, procedures, and regulatory guidelines.
  • 5% Participate in staff meeting and other agency activities as required.
  • 5% Will perform other duties as assigned.
  • Preferred Qualifications
    • Knowledge of the principles and practices of behavioral health care and co-occurring disorders, including assessment of risk and safety planning.
    • Knowledge and application of best practices in cultural competency, person-centered care, and Trauma-Informed Care.
    • Knowledge of laws, regulations, and practices pertaining to federal and state human service programs, specifically chapters 34, 51, 75, 92, 94.
    • Knowledge of human service and community resources and how to effectively utilize them.
    • Knowledge of Medicaid, Medicare, HMO's, Managed Care organizations, and public assistance programs.
    • Knowledge of psychotropic medications, their main effects, side effects, and abuse potential.
    • Knowledge of psycho-social rehabilitation services.
    • Ability to plan and organize work effectively.
    • Ability to take initiative, plan and organize work independently.
    • Ability to manage one's time and emotions in a fast-paced, often high-pressure environment.
    • Ability to maintain accurate, current records, and prepare clear and concise reports from them.
    • Ability to establish and maintain effective working relationships within the agency and community.
