Position: Human Services Professional I - Jail Reentry Specialist

Overview

Position Summary

The Jail Reentry Case Manager provides short-term and intensive case management to inmates with behavioral health needs who are reentering the community following incarceration. The Case Manager will begin working with identified clients while they are still incarcerated to adequately plan for the transition to the community, pro actively assessing needs and linking with supportive services. Upon reentry, the case manager will provide intensive case management and support to link with stable resources and long term treatment.

70% - Provide direct support services to assigned Jail Reentry clients.

Responsibilities
  • Establish relationships with incarcerated clients and assess client needs and risks associated with a return to living in the community.
  • Utilizes the ASAM to determine level of care.
  • Create a Service/Individual Recovery Plan for all clients served based on the assessment of needs and risks.
  • Implement case management services based on the assessment of client needs and risks. These services may include, but are not limited to: providing referrals to and coordinating services among community agencies, linking clients with employment, mental health and substance use treatment, health care benefits and other resources. Provide transportation to facilitate accessing needed resources, assisting clients with applications for public assistance programs, and assisting clients in obtaining and maintaining housing.
  • Coordinate the provision of medication assisted treatment for individuals returning to the community
  • Perform other direct services as assigned.
Professional Experience and Competencies
  • Engage in professional communication, consultation, and supervision as part of Jail teams; collaborate to work toward improved outcomes for clients.
  • Participate in supervision meetings and regularly consult with supervisors and colleagues to maximize the quality of services delivered.
  • Collaborate and consult with SUD internal and external service providers
  • Participate on community task forces and outreach efforts that support service delivery.
Program Compliance and Documentation
  • Ensure program and medical record compliance; complete data entry tasks as assigned to track outcomes for designated clients; follow all state, county and departmental policies and procedures; document client care in accordance with policies, procedures, and regulatory guidelines; attend program or departmental meetings or trainings as required.
  • Complete grant reporting and support grant writing as indicated.
Requirements, Education, Training & Experience
  • Graduation from an accredited college or university with a Bachelors degree in social work or related field required.
  • Must possess and maintain a current unrestricted driver’s license and auto insurance as required by policy.
  • Pass background investigation.
  • Minimum of one year of full-time experience working with adults who have severe and persistent mental illness.
Physical Elements and Working Conditions
  • Ability to stand, walk and sit; physical movement occurs a majority of the day.
  • Ability to enter client homes, including navigating stairs; both indoor and outdoor work with exposure to weather conditions.
  • Potential for contact with unpredictable and aggressive persons; potential for interactions with animals.
  • Must be willing to transport clients and be exposed to challenging environments; participate in disaster evacuations and fire drills; lift and carry up to 25 lbs occasionally.
  • Ability to use a computer and keyboard; become proficient with electronic medical records and MS Office within 2 months of hire.
Equipment and Scheduling
  • Ability to use a copy/fax machine and cellphone; flexible schedule including evenings as needed to meet client needs.
