Position Summary &Job Duties
Hours of work:
Week 1:
Sun, Wed, Thu, Fri;
Week 2:
Tue, Wed, Thu, Sat; 11 pm-7:30 am. Provides person-centered behavioral health assessment, care, resources and education to clients in need of crisis services. Supports emergency rooms, law enforcement, and other community agencies. Creates treatment planning, short-term treatment, and links to ongoing care. Pursues the least restrictive disposition that meets safety needs. Prioritizes community and crisis unit demands. Reviews and oversees safety plans, documents care, collaborates with internal and external colleagues, participates in clinical and administrative meetings, and supports the Human Services Department mission.
- Provide support and resources to crisis line callers.
- Assess clients to determine least restrictive disposition, including safety planning, voluntary or involuntary admission.
- Coordinate admissions and medical clearance with police, facility, and hospital ER staff.
- Develop person-centered safety and crisis response plans.
- Coordinate client care and linkage to ongoing behavioral health services.
- Facilitate detox facility admissions for individuals under 51.45 and provide post-discharge risk-needs assessments.
- Provide behavioral health risk assessment at the Rock County Jail and Youth Services Center.
- Knowledge of behavioral health principles, risk assessment, and safety planning.
- Knowledge of cultural competency, person-centered care, and trauma-informed practices.
- Understanding of federal and state human service regulations.
- Knowledge of community resources and public assistance programs.
- Knowledge of Medicaid, Medicare, HMO, Managed Care and psychotropic medications.
- Ability to plan, organize, and work independently in a fast-paced environment.
- Ability to maintain accurate records and prepare clear reports.
- Ability to establish and maintain effective working relationships.
- Ability to follow verbal and written instructions.
Graduation from an accredited college or university with a Bachelor’s degree in social work or a related field (Master’s preferred). Must work flexible hours, including weekends and evenings. Must possess an unrestricted driver’s license and auto insurance.
Physical elements: stand, walk, sit; navigate stairs; participate in disaster evacuations; lift up to 25 lbs.
- Computer and keyboard proficiency.
- Efficient use of electronic medical record system and Microsoft Office within 3 months.
- Use of copy and fax machines and cellphone.
- Work in office, client homes, and community locations.
- Exposure to weather conditions and unpredictable/aggressive persons.
- Potential interaction with animals.
