Act as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency and able to provide care following UPMC LPN Practice Guidelines and policies, and the State Board Regulations. The Licensed Practical Nurse (LPN) participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place under the direction of a Registered Nurse or other licensed UPMC provider.
Responsibilities- The LPN functions as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place.
- Communicate with a licensed professional nurse and the patient’s health care team members to seek guidance when: the patient’s care needs exceed the LPN’s scope of practice, exceed the LPN’s knowledge/skill, or the patient’s condition deteriorates or there is a significant change.
- Obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures. Follow the written, established policies and procedures of UPMC, UPMC LPN Practice Guidelines and the State Practice Act. Act within the LPN Practice guidelines (except IVs) and within the framework of Relationship Based Care to create a healing environment. Formulate daily goals and a plan of care with patient involvement, considering individual needs.
- Document comprehensively to promote communication between caregivers. Incorporate national professional standards and system goals to improve patient safety, quality and satisfaction. Build a caring, patient-focused experience by healing relationships with patients, families and colleagues. Actively participate in unit activities and assist care team members as needed.
- Provide detailed and appropriate teaching to patients and families, guide them through the episode of care and transitions to other levels of care or home. Seek feedback on performance and pursue continuous professional improvement. Support continuous learning in clinical areas by participating in organizational and system programs. Identify opportunities for clinical quality and workplace improvement.
- Develop and maintain productive working relationships internally and externally by demonstrating accountability, cultural awareness, and a balance between work and personal life. Model safe work hours and time management. Communicate safety hazards to peers and management.
Qualifications
Successful graduation from a Licensed Practical Nursing Program approved by the State Board of Nursing. The individual must be able to demonstrate the knowledge and skills necessary to provide care and interact with patients appropriate to the ages served, and to assess data reflective of the patient’s status. Knowledge of growth and development concepts and the ability to provide age-specific care are required.
Healthcare experience is preferred; prior LPN experience is not required. Ability to establish positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
